Key Practices for QAPI Success: In this highly interactive session participants will explore ways to strengthen their infrastructure for communication and problem solving among staff closest to the residents and with the interdisciplinary team. Learn how to build stability at the point of care by strengthening consistent assignments, and to create systems so staff have regular ways to share their first hand information about residents. Connecting the dots between bedside care and the conf rence room allows you to catch early warning signs and prevent avoidable declines. Shift huddles, QI huddles, LNA involvement in care planning, and management stand up with staff facilitate timely information sharing. Forty nine nursing homes participating in a Pioneer Network National Learning Collaborative incubated these practices and applied them to quality improvement efforts by building clinical interventions around residents’ customary routines. Individualizing care led to decreased rejection of care, and greater preventions of avoidable declines. Individualizing for one resident helped homes identify ways to transform systems to be more individualized for everyone rather than by exception.
