This intensive training session provides healthcare professionals with a targeted approach to identifying and correcting the most common documentation gaps in Minimum Data Set (MDS) assessments. Participants will learn strategic techniques to accurately capture patient acuity, improve case-mix classification, and optimize reimbursement under Vermont Medicaid and Medicare Part A’s Patient-Driven Payment Model (PDPM) guidelines. Special emphasis will be placed on the updated Appendix PP Guidance to Surveyors, effective April 28, 2025, under F641, which reinforces the critical importance of assessment accuracy and introduces new scrutiny for documentation practices. Compliance with the MDS 3.0 RAI Manual v1.19.1 coding guidelines will be comprehensively addressed, highlighting the escalating audit intensity from Medicare contractors and potential Medicaid audits, including complex validation processes and potential retroactive financial recoupments for documentation inconsistencies.
